Community Habilitation

Some children and adults with developmental disabilities are eligible for special services referred to as Community Habilitation Services. This program provides a worker who develops a detailed plan to teach one or more daily living skills and provides direct instruction at home or in a community setting. In other cases, an individual may identify a particular behavioral or social concern that places a strain on the individual or family and prevents full integration into the community. Summit staff can develop a plan to help the individual acquire the desired skills while providing valuable autism resources for parents.

Eligibility: This program is funded by the New York State Office for People with Developmental Disabilities (OPWDD). Certain eligibility requirements apply.
